Create a session in under 30 seconds
From the Consents list, click New session. Select the consent template, whether knee arthroscopy, colonoscopy, shoulder repair, or any custom template your practice has configured. Enter the patient name and MRN. The system sends an SMS and email invitation automatically.
You never touch the process again until the patient submits. At that point, you receive an in-platform notification and a countersignature request.

Track progress in real time
The Consents list shows every session at a glance. Status badges update automatically: Draft, In Progress, Submitted, Completed. Filter by status, search by patient name or MRN, and click through for full detail including the live audit trail.
If a patient has not opened the invitation after 48 hours, the platform flags the session for follow-up and sends a polite reminder automatically.

Countersign and complete
When the patient submits, you receive a notification. Open the session, review their answers and the comprehension check results, then apply your digital signature. The system generates the evidence pack immediately.
The completed PDF is filed to the patient record, the hash chain is sealed, and the session moves to Completed. Total clinician time: under 90 seconds.

Receive invitation
A short SMS and email arrive with a secure link. No login, no password. The link is authenticated by a one-time code the patient enters on their phone.

Read section by section
The procedure, risks, alternatives, and what to expect are each presented as a separate section. Plain language. No medical jargon without explanation.
Each section ends with a comprehension check: one question, four options. The patient must answer correctly before advancing.

Sign and submit
The patient draws their signature on the phone screen, reviews their answers one final time, and submits. The evidence pack is generated immediately.
If the patient cannot finish in one sitting, their progress is saved. They return via the same secure link and continue where they left off.
Output
The evidence pack
Every completed session generates a tamper-evident PDF evidence pack. It contains the patient's answers, the comprehension check results, both signatures, and a full audit table with event timestamps and SHA-256 hashes.
The hash chain in the audit table means any tampering with the record after generation is mathematically detectable. The pack stands as valid evidence in medico-legal proceedings.
